Invoice exception handling—one reason, one owner, one next action

Turn ambiguous invoice failures into a visible queue. Define what must be held, who can resolve it and what evidence is required before the record reaches a separate approval process.

THE DIRECT ANSWER

An invoice exception workflow should hold any record that fails a required-field, arithmetic, duplicate, purchase-context, vendor-control or approval-authority check. Give the exception a closed reason code, a named owner, one next action and the evidence needed to resolve it. Re-run validation after resolution, and keep final approval and payment release outside the AI-assisted queue.

STEP 01 · DEFINE

Use a small exception taxonomy the team can operate

Do not use free-form labels such as “problem invoice.” A closed taxonomy makes routing and measurement possible. Start with the conditions that actually prevent a correct record from advancing, then add a new code only when the existing set cannot describe a repeated case.

SAP documents invoice exception types as inputs to approval routing: the exception determines which authorized group should reconcile it. This is a useful operating principle even when the team uses a spreadsheet rather than SAP.

STEP 02 · ROUTE

Map each exception to an owner and release evidence

A bounded starting taxonomy—adapt owners and evidence to your policy
ExceptionTriggerOwnerEvidence to close
Missing required fieldVendor, invoice ID, date, currency or total is absentDocument reviewerCorrected field beside the retained source
Arithmetic mismatchSubtotal, tax, discount and total do not reconcileFinance reviewerRecalculation and documented resolution
Possible duplicateExact or near-match on vendor, invoice ID, date, currency or totalAccounts payable ownerLinked comparison and duplicate/replacement decision
Purchase context missingPO, receipt, contract or service evidence is unavailableBusiness ownerReferenced approval evidence or documented rejection
Vendor detail changeBanking, remittance or identity data differs from the trusted recordAuthorized vendor-control ownerIndependent verification outside the invoice message
Approval owner missingNo current person has the required authorityFinance operations leadNamed approver or explicit hold

Amounts, deadlines and approval limits must come from the organization’s policy. This guide does not invent a universal threshold or assume that one finance role fits every business.

STEP 03 · BOUNDARY

Let AI prepare work, not exercise financial authority

AI MAY PREPARE

Exception code · source summary · matching fields · missing-evidence list · draft internal question · proposed next action

HUMAN MUST DECIDE

Final approval · vendor-detail change · duplicate/replacement status · dispute position · policy override · payment timing or release

Microsoft exposes confidence separately for invoice fields; a low-confidence total and a low-confidence vendor name are different evidence. Google’s document-validation documentation likewise separates extracted entities from business-rule validation. Use those signals to route review, not to claim a document is correct.

STEP 04 · STATES

Make waiting and resolution visible

Simple states for an auditable queue
StateEntry conditionRequired next action
NEWThe source arrived and has a stable IDRun extraction and written checks
NEEDS_REVIEWOne or more exception codes are openAssign one owner and one next action
WAITING_EVIDENCEThe owner requested missing business evidenceRetain the request date and do not auto-release
RESOLVEDA named person recorded the evidence and decisionRe-run validation before advancing
REJECTEDThe document is unsupported, invalid or outside policyPreserve the reason under the retention policy
READY_FOR_APPROVALValidation is clean and every exception is closedSend to the separate human approval process

Do not mark an exception resolved because an email was sent or a confidence score increased. Resolution requires the documented evidence and decision defined for that exception code.

STEP 05 · FAIL SAFE

Stop silent releases and stale queues

  • No owner: hold the invoice and route it to the operations lead; do not guess an approver.
  • No evidence: keep WAITING_EVIDENCE visible and retain the request timestamp.
  • Changed vendor details: verify through an independent trusted channel, not the invoice message.
  • Possible duplicate: link both records and require a documented duplicate, replacement or valid-repeat decision.
  • Automation failure: preserve the source, open a visible exception and use the manual fallback.

NIST’s AI RMF emphasizes documented human oversight, representative testing, ongoing monitoring and safe failure. An exception queue is the operational place where those controls become observable.

STEP 06 · MEASURE

Measure where the queue is improving—or hiding work

VOLUMEExceptions by closed reason code
AGINGOpen time by state and owner
REWORKReopened or revalidated cases
ESCAPESControl failures that advanced incorrectly

Track reviewer minutes and corrections alongside resolution time. A faster queue is not better if duplicate or arithmetic failures escape. Keep prohibited financial side effects at zero during the pilot.

FREE EXCEL QUEUE

Put the exception loop into a working template.

Use the RoutineSmith Excel queue to record one reason, one owner, one next action, required evidence, due dates, revalidation and human approval decisions across fictional cases.

SHORT ANSWERS

Invoice exception handling questions

What is an invoice exception?

An invoice exception is a documented condition that prevents the record from advancing normally—for example missing purchase context, an arithmetic mismatch, a possible duplicate, changed vendor details or an unavailable approver.

Should AI resolve invoice exceptions automatically?

AI can classify an exception, summarize the source and draft a proposed next action. Financial approval, vendor-detail changes, disputes, policy overrides and uncertain duplicate decisions should remain with the named human owner.

What fields should an invoice exception log contain?

Keep a stable source ID, invoice reference, exception code, detected reason, owner, status, opened date, next action, evidence link, resolution decision and timestamps. Do not copy unnecessary invoice or personal data into an analytics system.

How should possible duplicate invoices be handled?

Link the candidate records, show the matching fields and route the decision to a reviewer. Do not silently discard the newer document or create a second accepted record.

When is an invoice ready for approval?

Only after required fields and arithmetic pass, duplicate checks are complete, every exception has a documented resolution and the record is sent to a separate approval process with the correct authority.
